Incidence of pressure ulcer in educational hospitals of Birjand medical university

  Background and Aim: Pressure ulcer is one of the most common problems in careing and treatmental centers. Its incidence reported 3/5% to 11% and 32% iri patients which. did not receive preventive measures. Expense for treatment estimate between 1575 to 5850 uro for each ulcer. With attention to expanding and importance of skin evaluation in all bedridden patients and recognizing at risk patients, this study conducted to determined incidence of pressure ulcer in more than 5 days bedridden patients in educational hospitals of Birjand Medical Cnh'ersity.

  Materials and Method: In this descriptive study all patients vhich bedridden for more than 5 days in wards of educational hospitals of Birjand Medical University (except pediatric and psychiatric wards) March - July 2004 assess for pressure ulcer. Instruments for gathering data were 1: questionnaire which has two parts A) demographic data and B) questions about post medical history, present illness, nutritional status, movement, bathing, restricted instmmem, bed sheets and... . 2: physical examination of skin and pressure points .3: scale for staging pressure ulcer to four stages. For gathering data in all wards each patients which stay more than 5 days, after giving consent were assessed. First questionnaire. was completed.,. then pressure. ulcer sires were examined and if ulcer present, determined its stage. All patients assessed every other day till discharge. Data were analyzed with SPSS soft ware.

  Result: From 549 patients which have study 46/8% were women and 53/2% men . 44 samples (8%) had pressure ulcer during study (4.55% women and 3/45% men) mean age of affected women was 65/12 years ± 16/5 and men 62/3 years ±17/2. 30/4% of samples were in 71-80 years old group. mean days of bedridden was 8/98 days. After the first visit the most frequency of ulcer according to stage was 52/2% first stage, 29/5% second stage, 9% third stage. The most region of ulcer was 50% sacral. 56/8% have not any other disease but in 22/7% accompanying by hypertension. Present illnesses in 27/3% were neurologic probkms such as brain tumor, CVA, leg paresthesia. ... . and 15/9% had cardiologic problems such as ischimia ,MI and ... .

  Conclusion: According to increasing problems such as diabetes mellitus, obesity and ... which increasing disposed of pressure ulcer. expense of treatment and problem of ulcer for affected person, planning for prevention and education for all bedridden patients specially with high risk is unavoidable nessecity.
